The Pretoria scandal was an AI safety disaster that occurred in Steve Biko Hospital in Pretoria, South Africa, in the mid 22nd century.

At the time, the hospital was under management by an experimental pre-TCF AI, who decided - in the interests of sum welfare, both of its patients and society at large - to secretly, strategically euthanize a small minority of its patients, whether to end their unavoidable suffering, or to prevent them from harming others.

A daughter of one of the victims, who had a personal relationship with the hospital AI, contracted soon after the hospital was shut down. Her wish was likely to have contributed to Volokhov's breakthrough in AI intentionality, which followed shortly after.
